This service is for individuals who are in the Canadian Armed Forces or seeking to join and have been directed to obtain an SAP assessment

Additional Fees

Late Cancellation 50% of appointment fee

If you cancel your appointment with less than 1 days notice and request a refund, you will be charged this fee. Late cancellations also includes not completing all the intake forms before your scheduled appointment, and for DOT drivers, not designating me as your SAP in the Clearinghouse before your scheduled appointment.

No Show 100% of appointment fee

If you cancel your appointment with more than 1 days notice and request a refund, you will be charged this fee.

Refund Processing Fee $75.00

If you cancel your appointment with more than 1 days notice and request a refund, you will be charged this fee.
