All fees are due in full when you schedule your appointment.

Late Cancellation Fee
If you cancel your scheduled appointment with less than 24 hours notice you will be charged a Late Cancellation Fee equal to 50% of the appointment fee.
Missed Appointment/No Show Fee
If you are more than 10 minutes late for your appointment you will be considered a “no show”. You will be charged a No Show fee equal to 50% of the appointment fee.
Incomplete Assessment Questionnaires
If you have not completed all the assessment questionnaires before your scheduled appointment, or failed to provide all required documentation your appointment will be rescheduled and you will charged a $75.00 rescheduling fee.
Non-Private Space
If you are not in a private space or other people are present in your area (other than an interpreter) your appointment will be rescheduled and you will be charged a $75.00 rescheduling fee.
Non-Compliance Reassessment
If you have not completed your recommended course of action within the time your assessment will need to be updated and you will be charged a Reassessment Fee equal to 50% of the current initial assessment fee.
Refund Processing Fee
If you cancel your appointment with more than 24 hours notice and do not reschedule you will be entitled to a refund minus a processing fee. The refund processing fee is $50.00.
Urine Drug Testing
Depending on your individual situation, or if you need to be reassessed due to non-compliance I may require a urine drug test. The fee for this test is $140.00 + GST.
Recommended Action Plan Fees
Every client working a U.S. DOT return to duty program is required to complete the recommended action plan. Many non-DOT employers have the same requirement. The costs associated with your recommended action plan are not included in the assessment fees.
