Overview of Services
General Information:
    • All intake forms and questionnaires must be completed 24 hours before your scheduled appointment. If you have not completed them your appointment will be cancelled and your referral source must authorize another visit.
    • As a SAP, I only offer assessment and referral services. I am not permitted to provide therapy services to assessment clients. Additionally, I cannot refer you to any person or organization from which I receive payment, or to any person or organization in which I have a financial interest.
    • As an SAP, I am prohibited from considering certain factors when making recommendations. These factors include: employee claims the employer’s workplace policies are unjust or poorly administered, employee claims the testing process was unjust or inaccurate, or employee attempts to downplay the seriousness of the workplace policy violation.
Intake Forms, Screening Questionnaires, and FMCSA Designation
    • The required intake forms and questionnaires can take up to 60 minutes to complete. They must all be completed before your scheduled appointment or your appointment will be cancelled and you will be charged a late cancellation fee.
    • If you are a US DOT regulated driver you must designate me, Jim Mason with ClaritySAP as your SAP in the FMCSA Clearinghouse.  Failure to do so will result in your appointment being cancelled and a late cancellation fee charged to your account.
Cancellations, Late Cancellations, and Missed Appointments:
    • If you cancel your appointment with less than 24 hours notice, for any reason, you will be charged a late cancellation fee equal to 50% of your appointment fee.
    • If you have not completed all required by your appointment time, your appointment will be cancelled and you will be charged a cancellation fee equal to 50% of your appointment fee.
    • If you do not show up for your session your will be charged 100% of your appointment fee.
Refund Processing Fee:
    • If you cancel your appointment and request a refund, you will be charged a $75.00 refund processing fee.  This will be automatically deducted from the amount of your refund.
No Pre-determined Outcomes:
    • I do not determine the outcome of your assessment in advance.
    • The recommendations are based on the answers you provide on the forms and questionnaires you complete, as well as information collected during the face-to-face portion of the assessment.
Limited Duration of Services
    • Your involvement in the SAP return to duty process is voluntary. You can stop receiving services at anytime.
