You can start your assessment by clicking the button below. These are quite a few questions. You’ll find a progress bar at the top of each page indicating your progress.
The form includes a “Save and Continue option”. “Save and Continue” temporarily saves your work and allows you to continue the questionnaire at a later time. If you use this option a link will be sent to you in email. It will be sent to the email you provided in the questionnaire.
The SASSI-4 is a separate question. It is not included here. You’ll receive another email with a link to the SASSI-4 directly from the SASSI Institute.
For me to have time to review all the questionnaires, you must complete all of them, including the SASSI, at least 1 business day before your appointment.
If you have not completed all the assessment questionnaires before your scheduled appointment, per the published fee schedule your appointment will be rescheduled and you will charged a $75.00 rescheduling fee.

Virtual/Remote sessions are conducted via ZOOM.
For the assessment interview, you must be in a private location at home or office (private office, conference room, etc). If you are in any other location (non-private space, vehicle, outside, restaurant, etc.). Except for an interpreter, you can not have another person in the area.
There are six steps in the Return to Duty process
Step 1: Substance Abuse Professional (SAP) Evaluation
The evaluation consists of online questionnaires and a face-to-face substance abuse evaluation. The face-to-face portion is done over Zoom and takes approximately 60 minutes.
Step 2: Complete the Recommendation Course Action
Based on the results of your assessment you will be referred to education, counselling, outpatient treatment, or in-patient treatment. Recommendations are mandatory for all US DOT drivers. Many, but not all companies also require non-DOT workers to complete the recommendations.
Drug Testing
After consulting with the Medical Review Officer who determined the results of your drug test I may require you to undergo drug testing as part of the Recommended Course of Action. This is not Return to Duty testing. The purpose of this test to verify compliance with your Return to Duty program and your eligibility to take a Return to Duty test.
Step 4: Complete a Follow-up SAP Evaluation
After I receive written confirmation that you’ve completed all the recommendations, I’ll authorize you to schedule your follow up assessment. The purpose of this face-to-face assessment is to evaluate your compliance and determine your eligibility for Return-to-Duty testing.
Step 5: Return to Duty Testing
Only your current or potential US DOT employer can send you for a Return to Duty test. Return to Duty tests must be done under direct observation.
If your Return to Duty test is positive you will be required to restart the Return to Duty process.
Once your employer enters your negative Return to Duty test results into the Clearinghouse, you are legally cleared to drive commercial vehicles into the United States again.
Step 6: Follow-up Drug & Alcohol Testing
The DOT requires I provide your employer with a follow-up alcohol/drug test schedule. Follow-up testing can last for five years, and each follow-up test must be under direct observation. Changing employers will not make the follow-up test schedule go away.
