The following information can help you identify the type of service you need to schedule
Select SAP Assessment if you are truck driver or in another safety-sensitive role.
If you are a US DOT regulated driver, you must designate me, Jim Mason with ClaritySAP as your SAP in the Clearinghouse before scheduling your assessment.
Select FAA/Transport Canada if an FAA or Transport Canada offical has instructed you to obtain an SAP assessment.
Select Canadian Armed Forces if you are a member of the armed forces or seeking to join and have been instructed to obtain an SAP assessment.
Select Follow-up Session only if you have completed all of the following:
- You have completed all the recommendations.
- You have provided me with a certificate of completion for the recommendations.
